Does the Max Resolution thumbnail always exist?
No. The maxresdefault.jpg thumbnail (1280×720) is only generated for videos that were uploaded in HD (720p or higher). Older videos or those uploaded in SD may only have the hqdefault.jpg (480×360) as their highest quality thumbnail. If a thumbnail fails to load, it means that size is not available for the selected video.
